We ask each recipient for a brief evaluation of the impact of our funds, due six months to one year after receiving a grant from the Community Foundation (individual grant due dates are stated on the grant notification letter). These reports are a valuable tool that allows us to measure the effectiveness that our grantmaking has in building a vibrant community. Please note that future grants may be denied if the evaluation is not received.
On a separate sheet of paper, please respond to the following questions, using no more than TWO pages.

1. Did you meet the goals you set out to achieve? Explain why/why not. Please include any statistical evaluation data that you use to monitor organization/program effectiveness (i.e. number of people served, empowered to take action, changed by your work, etc.).
2. What did you learn? Will you be making any changes due to the learnings?
Community Question:
3. From your perspective, have you noticed any community-wide changes/trends in terms of need or the population you serve? These changes or trends (for the positive or negative) do not necessarily have to be as a result of your direct intervention efforts.
